General Information


Thank you for choosing the Department of Radiation Oncology at UCSF. In order to give you the smoothest patient experience, we ask that you complete the new patient paperwork prior to your initial visit.

At your first visit you will need to bring your health insurance card(s), a photo ID, and your new patient paperwork (available for download here):

For General New Patients:

You may return your paperwork to us via email to RadOncNewPatient@ucsf.edu, fax (415) 353-9883, or print it out and bring it to your appointment.

You can also complete your forms through UCSF MyChart.

MyChart also allows you to:

  • Send messages to your health care provider
  • Refill a prescription
  • Check lab results and graph your trends
  • View electronic health information
  • Request an appointment or a referral
  • Access your child's medical records if your child is 11 years old or younger
  • Access some of your adolescent's medical records if your child is 12 to 17 years old
  • View information about past visits and upcoming appointments
